Cardiovascular disease (CVD) is a general term for conditions affecting the heart or blood vessels.

CVD is one of the main causes of death and disability in the UK, but it can often largely be prevented by leading a healthy lifestyle. Learn more.


Why you have been invited

You will be invited annually by the practice for a cardiovascular review if you fall on one of the following disease registers and have outstanding monitoring/investigations:

What happens in the appointment

The nurse will talk to you about your symptoms and how you are managing with your condition.

You may have investigations, such as bloods, blood pressure and urine.

You will be asked to talk about your diet & lifestyle habits, and review healthier changes you can make to prevent your disease worsening.

Treatment options, such as medications, may be offered if it could help to improve your condition.

How to book or decline the appointment

If you have been sent a link in a text message, you can use this to easily book your appointment online at a location, date and time that suits you best.
