Why you have been invited
You will be invited annually by the practice for a cardiovascular review if you fall on one of the following disease registers and have outstanding monitoring/investigations:
- Coronary heart disease (CHD)
- Chronic kidney disease (CKD)
- Stroke or Transient ischemic attack (TIA)
- Peripheral arterial disease (PAD)
What happens in the appointment
The nurse will talk to you about your symptoms and how you are managing with your condition.
You may have investigations, such as bloods, blood pressure and urine.
You will be asked to talk about your diet & lifestyle habits, and review healthier changes you can make to prevent your disease worsening.
Treatment options, such as medications, may be offered if it could help to improve your condition.
