Enhanced Health Check

The Enhanced Health Check is very similar to a standard NHS Health Check, in that it aims to improve your health and wellbeing.

We are all at risk of developing health conditions that may affect us in the future, but spotting things early can often help us to help put preventions in place to help us live in good health for longer. The purpose of the Enhanced Health Check is to find things early so that we can make the right offer of support to you to help prevent things developing further.

The standard NHS Health Check can help you prevent heart disease, stroke, diabetes, kidney disease, and other conditions that we become more at risk of developing with age.

The Enhanced Health Check does all of the above, as well as further screening, vaccinations, and your mental and physical wellbeing.

Across Lancashire and South Cumbria, there are some residents who qualify for a NHS Health Check, and who we believe would benefit from further screening and support, which is why they are being offered an Enhanced Health Check.

Frequently Asked Questions

We recognise that things in your day to day life can affect your health so an Enhanced Health Check gives us the opportunity to have a chat to understand if that is the case with you and if we can do anything to help.

We will ask you some questions about your current circumstances and lifestyle. We will also measure your height and check your weight, take your blood pressure and do a blood test. It shouldn’t take longer than an hour although it may have to be completed in 2 parts on different days.
Across Lancashire and South Cumbria though there are some residents who qualify for the standard NHS Health Check (those aged 40 – 74 with no known health conditions), and who we believe would benefit from further support, which is why they are being offered an Enhanced Health Check.

To begin with we’re focusing on people aged between 40 – 74 with mental health issues or those who live in an area where a lot of people are socially vulnerable (for example those who live in poverty, have lack of access to transportation, or live in crowded housing). GP practices may choose to invite others though who they also think would benefit.
If you have had an Enhanced Health check, you will be invited every 5 years for another one, if you are still eligible.

If you’ve been invited and not responded, or declined, we will aim to invite you again every year encase you’ve changed your mind.

Blood pressure checks

A blood pressure test checks if your blood pressure is healthy, or if it’s high or low.

You should have a blood pressure test if you’re worried about your blood pressure at any time.

If you have been diagnosed with high or low blood pressure, or you have a high risk of developing either, you may need more frequent checks of your blood pressure. Learn more.

Non-urgent advice: Aged 40 and over?

If you’re aged 40 and over, and are eligible, you may be able to get a free blood pressure check at a local pharmacy.

Blood tests

If you’ve been asked to have a blood test, you should book an appointment.

You may be asked to have a blood test if:

  • the clinician requests investigations for you
  • you’re on certain medications where you blood levels should be monitored
  • you have a medical condition that requires regular monitoring of your blood levels
If you have been told you need to fast for your blood test, then you should only consume water from 10pm the night before, and aim to book an early morning appointment for your test.

It is important that you remain well hydrated before you attend for your blood test.

COPD reviews

Chronic obstructive pulmonary disease (COPD) is the name for a group of lung conditions that cause breathing difficulties, including emphysema and chronic bronchitis.

COPD is a common condition that mainly affects middle-aged or older adults who smoke. Many people do not realise they have it. Learn more.


How often you should have a review

You should have a review at least once every year if you are living with COPD.

The nurse may also request you have interim reviews to see how you’re managing on your current treatment plan where necessary.

What happens in the appointment

In the appointment, you may:

  • talk about your symptoms – for example, if they’re affecting your normal activities or are getting worse
  • discuss your current treatment– including if you think you might be experiencing any side effects and if you need to be reminded how to use your inhaler
  • be offered tests to monitor your health

It’s also a good chance to ask any questions you have or raise any other issues you want to discuss.

Asthma reviews

Asthma is a common lung condition that causes occasional breathing difficulties.

It affects people of all ages and often starts in childhood, although it can also develop for the first time in adults.

There’s currently no cure, but there are simple treatments that can help keep the symptoms under control so it does not have a big impact on your life. Learn more.

Travel health clinic

The practice offers a travel health clinic with some of our nursing team to provide some vaccinations which are free on the NHS.

This service is for patients of Bloomfield Medical Centre only. This is not a private service.

Check if you need vaccinations

If you’re planning to travel outside the UK, you may need to be vaccinated against some of the serious diseases found in other parts of the world. Learn more.

You can find out which vaccinations are necessary or recommended for the areas you’ll be visiting on the NHS Fit for Travel website.

You should provide 4 to 6 weeks notice before you travel.

Annual health check for people with severe mental health conditions

Anyone aged 18 or over who has schizophreniabipolar disorder or psychosis can have a free health check once a year.

It is up to you to decide if you want to have an annual health check, but these checks can make a big difference to your health and wellbeing. Learn more.


What happens in the appointment

  • A physical check-up will be offered, including weight, heart rate and blood pressure.
  • Investigations, such a blood and urine tests, may be carried out.
  • Discuss how to stay fit and healthy.
  • Review your mental health care plan.

Cardiovascular reviews (Complex)

Cardiovascular disease (CVD) is a general term for conditions affecting the heart or blood vessels.

Why you have been invited

You will be invited annually by the practice for a cardiovascular review if you fall on one of the following disease registers and have outstanding monitoring/investigations:

What happens in the appointment

The pharmacist will talk to you about your symptoms and how you are managing with your condition.

You may have investigations, such as bloods, blood pressure and urine.

You will be asked to talk about your diet & lifestyle habits, and review healthier changes you can make to prevent your disease worsening.

Treatment options, such as medications, may be offered if it could help to improve your condition.

General wellbeing assessment

You’ve been invited to book a telephone appointment with a Care Coordinator so that we can understand more about you’re current general wellbeing and how you’re managing with your day-to-day life. This is a holistic assessment.

As this is a new service, we understand you may have a few questions, so please continue reading to find out more.

What happens in the appointment?

In this telephone call, you’ll be asked questions about your day-to-day activities and general wellbeing, this includes:

  • independence in daily actives
  • mobility and falls risk
  • mental wellbeing
  • diet and lifestyle

This telephone call should last somewhere between 10-20 minutes, depending on your current health and needs.

What is the reason for this appointment?

We want to make sure that you’re getting any and all of the support you many need for any unmet needs you may have at the moment.

Even if you’re feeling fit and well, or feel you have no unmet needs, we strongly encourage you to book in.

It is also a chance for us to keep your record updated and inform you of any appointments, checks or monitoring you may be due to have.
